Media Consent

The Stigma-Free Mental Health Society (the “Society”) collects personal information about its community
members in the form of stories, photographs, videos and other media. The Society shares and translates
that information on our website and social media pages, in printed publications, at public presentations and
internally within the organization. The Society collects this information in order to promote the Society and
its initiatives, to highlight achievements of community members, and to support education and staff training.
The Society may also share this information with its promotional partners within Canada, including but not
limited to television services, for distribution of the Society’s programs, stories, photographs, videos and
other media, on a video-on-demand basis, made available to the public and/or customers of the Society’s
promotional partners. In particular, the programs publicly offered by the Society may, from time to time, be
televised or otherwise made public through the Society and its promotional partners.

The Society follows British Columbia’s Personal Information Protection Act (“PIPA”) in the collection of
personal information. By signing below, you are agreeing that you have read the information contained in
this document and that you give consent for the Society to collect, translate as necessary, and use your
personal information in the ways described above for no cost or compensation or claim of rights. You give
consent that your personal information may be stored and viewed inside or outside of Canada. You grant
to the Society a non-exclusive, irrevocable, unrestricted, royalty-free license to record, reproduce, translate,
transmit, and display the personal information as described above, and you agree that the Society may
alter or modify that information or combine it with other media, including but not limited to other images,
text, audio recordings and graphics.

Your consent is considered valid for current and future use of the personal information you have provided.
If you want to withdraw your consent, you can contact the Society by telephone at 778-678-2223, or by
email at [email protected] If you withdraw consent, this will only apply to any
future use of your story, photo, video and/or other media. Your personal information may continue to appear
in promotional videos, for example, that were created when your consent was in effect.
